Register as New Patient

If you live within the practice boundary (see map below) and need to register with a GP, please complete the registration and patient history forms.  This will provide us with the necessary information to help us look after you until your records arrive from your previous GP.

You can print off the forms below and bring them into the surgery to complete your registration.  Alternatively, you can register on line by clicking here.

Everyone is welcome in general practice.  You do not need proof of ID, address, immigration status or an NHS number in order to receive care or see a GP.  

It is not possible to register with a particular doctor as all registrations are with the Practice, however you may express a preference to receive care from a particular GP. This request will be held on your medical records and we will do our best to accommodate you in this.

If you change your address within the Practice area (see map above), your telephone number or other details, please notify the Practice in writing as soon as possible.  You can do this online from the home page.

Please note that the Practice does not accept out of area registrations.

Anyone who moves outside of the practice boundary will be removed and will need to register at a new practice.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
