How does it work?
Your needs are written in your health record. A flag shows staff you need support.
With your permission, this can be shared with other NHS services.
How do I get a flag?
1 Talk to your GP, nurse, or staff member
2 We will talk about your needs
3 We will record them in your notes
Can I change it?
1 Yes, you can update your needs
2 You can remove the flag
3 We may check your needs at appointments

You will be asked before your information is shared.
You can say yes or no and change your mind at any time.
