The Reasonable Adjustment Digital Flag tells the NHS what support you need. It helps staff
understand how to care for you.


What is a reasonable adjustment?

A reasonable adjustment is a change to help you get care.
1 More time at your appointment
2 Information in easy read or large print
3 A quiet waiting area
4 Help with communication

Why is this important?

1 Staff know your needs
2 You do not have to explain again
3 Your care is safer and easier


How does it work?

Your needs are written in your health record. A flag shows staff you need support.
With your permission, this can be shared with other NHS services.


How do I get a flag?

1 Talk to your GP, nurse, or staff member
2 We will talk about your needs
3 We will record them in your notes

Can I change it?

1 Yes, you can update your needs
2 You can remove the flag
3 We may check your needs at appointments


Your choice

You will be asked before your information is shared.
You can say yes or no and change your mind at any time.
